Amylin and Cagrilintide

Amylin is a 37-amino acid peptide that is released with insulin from pancreatic beta cells when nutrients are delivered to the small intestine. The peptide binds to a complex receptor consisting of the calcitonin receptor and one of the receptor activity-modifying proteins (RAMPs) that is widely distributed in the brainstem, with concentrations in the area postrema and the arcuate nucleus of the hypothalamus.

Amylin is a hormone produced by pancreatic beta cells and secreted in conjunction with insulin. Cagrilintide is a synthetic acylated amylin analog. It is currently available synthetically at the Omizzur Peptide Laboratory.

The focus of cagrilintide is not monotherapy but in combination with semaglutide. Amylin analogs (cagrilintide) are being developed in a fixed-dose combination with the GLP-1 analog semaglutide for the treatment of obesity. CagriSema is a combination of 2.4 mg cagrilintide and 2.8 mg semaglutide.

The results of a phase 2 randomized controlled trial showed that after 32 weeks of treatment with CagriSema, 2.4 mg cagrilintide, and 2.8 mg semaglutide, the average HbA1c of patients with diabetes and obesity decreased by 2.2, 0.9, and 1.8 percentage points, respectively; the average body weight decreased by 15.6%, 8.1%, and 5.1%, respectively.

Studies have found that infusion of amylin can reduce food intake in healthy individuals and patients with type 1 diabetes (T1DM). Although the level of amylin in obese patients is higher than that in non-obese controls (which may indicate the presence of amylin resistance), synthetic amylin analogs have been successfully used to treat obesity in clinical trials and may have a synergistic effect with the appetite suppressant leptin.
